F02 — Consent to Be Processed

Before we may examine your file, applicable data-protection regulations require your explicit, informed consent to processing. Since 2019 this department has additionally required that consent be a matter of public record: a tick alone is not evidence of anything. You must therefore publish a document containing the words I AGREE, in capital letters, at one of our approved publication services, and enter its address below. We will retrieve and read it. Your consent reference will be emailed to you as a confirmation of intake and is required by subsequent departments.
